Yes, Yad Vashem maintains these forms filled out by surviving family
members or by friends.
The first time I went to a genealogy conference (July, 2000), Yad Vashem
had a booth and I talked to their representatives there.
They were giving out forms to be filled out. I took a whole bunch home with
me and mailed 22 completed forms to them the next week.
As I have collected so much material on my family over the years, I was
able to do this.
Two things to note: a researcher looking through their records, either in
person or online, will sometimes see 2 or more pages of testimony on the
same person - this resulted from several people sending in pages without
the others knowing.
One important item: you can now download the blank form and print it out
or mail it in or you can fill out a blank form online and email it to them -

And yes, the US Holocaust Memorial Museum has amazing collections
and they are very helpful to those email, phone or visit them.
